Health Coach
JBS STRONG is an evidence-based, best-practice worksite health coaching program that Identifies and engages employees with multiple chronic conditions who are at the greatest risk for disabling, life-threatening, and costly health problems The JBS STRONG health coach:
- Encourages employees with chronic conditions to improve their health (via motivational interviewing)
- Guides employees through a structured sequence of evidence-based behavioral interventions aimed toward improving motivation and skills that will facilitate enduring change
- Connects employees with JBS clinics and essential community resources for health improvement
- Supports employees continuing efforts through ongoing groups and follow-up sessions
Key Result Areas
- Work at JBS or Pilgrim’s Pride worksite as on-site health coach for members with chronic conditions
- Conduct member screenings for various health risks
- Conduct member assessments of health status, health engagement and health risks
- Develop member-centered treatment plans with members, provide education materials and ensure quality, cost effective care recommendations
- Evaluate effectiveness of care plan and progress made by member
- Engage employees in skill-building, goal-setting, resiliency-building, and healthy living skills
- Facilitate health education and peer support process groups
- Coordinate care with the clinical care team at off-site clinic
- Chart interventions and member progress in case management system and Excel spreadsheets
- Coordinate care with Case management and Utilization Management staff
- Collaborate with employees and physicians to formulate an optimal plan of care that meets all of the medical, psychosocial and work-life needs of the member
Minimum Qualifications:
- English-Spanish bilingual required
- 2+ years clinical or community health experience working directly with patients required
- Motivational Interviewing training and experience preferred
- Health coach certification preferred, but not required, as we will co-sponsor your certification program
- Bachelor’s or Master’s degree in health science, psychology, behavioral sciences, public health, counseling, social work, or nursing preferred
- Candidate should have some level of certification or licensure in one or more of the following healthcare occupations: nursing, counseling, social work, health coaching, medical assistant, or some clinical field relevant to chronic care management. Some examples include, but are not limited to: RN, CCP, RHC, CMA, LPN, LVN, LCSW, LPC, LCDC, LAC, CHES
- Candidate should have strong experience working with people to improve their health
- Candidate should be health-centered and should be able to demonstrate healthy lifestyle skills
- Demonstrates integrity, initiative, good judgment and teamwork
- Strong interpersonal relationship & communication skills, and the ability to work autonomously
- Intermediate skills with Microsoft Office, with the ability to navigate a Windows environment
- Time management, organizational, research, analytical, and documentation skills.
- Ability to work independently, efficiently, perform multi-tasking, prioritize tasks, and meet deadlines.
