Summary

Promote the overall health and wellness of the client population through improved health and productivity management. Develop, implement and manage wellness program initiatives and events that create awareness and motivation to help employees adapt and maintain a well-rounded healthy lifestyle. 

Essential Responsibilities

Develop, coordinate, manage and maintain total well-being programming to reduce health risk, medical costs and encourage healthy lifestyles

Case Management of VSR (Vital Systems Review) Program

Manage MHO’s wellness platform to include adding content, patient promotion, running reports, etc.

Select and manage vendor partners and to ensure performance

Seek out community resources that would be beneficial to the patient’s wellness goals

Create well-defined metrics to measure the success of the programs

Stay on top of any changes to health and safety regulations with government agencies

Educate patients regarding their illness and treatment, including self-care, prevention and health maintenance

Obtain and record vital signs (BP, heart rate, pulse oximetry, height, weight, waist circumference)

Perform all nursing care duties, including procedures, injections, EKG’s and phlebotomy

Assist the physician and medical staff with procedures and treatments

Document all assessments, actions, care and treatment responses, and communications in patient record in EMR

Maintain patient confidence and ensure confidentiality of patient care information

Minimum Qualifications

Education: Associate’s degree in Nursing, BLS for Healthcare Providers 

Experience: At least 2 years of experience, preferably in primary or urgent care setting

Licensure: Current and unrestricted RN State of Florida license

Preferred Certifications: Certified Nurse Educator (CNE), Certified Diabetes Educator (CDE), Tobacco Treatment Specialist (CTTS)

Knowledge/Skills/Abilities

Understand CPT and ICD9/ICD10 Codes

Proficient knowledge of computer systems including electronic medical record (EMR)

Understanding of public health issues and employee health and wellbeing trends

Ability to effectively communicate with staff and patients using excellent written and verbal skills

Friendly personality and ability to work well as a team member
