Nurse Health Educator

Sterling Wellness Solutions is looking for a Primary Care Nurse to service account in New Jersey and travel to other locations biannually, including Tennessee, Florida, Michigan, and Ohio. Duties include:

• Provide health and wellness services during onsite visits including:
• group seminars
• scheduled individual consultations
• blood pressure checks
• measure weight
• assist participants with completing online health risk appraisals
• general health education
• promote online health challenges
• assist with benefit plan questions and/or concerns
• Complete telephonic health consultations as determined by the annual program schedule
• Chronic care outreach
• Review health status
• Review annual wellness screening results and make physician referrals as needed
• Set realistic goals
• Monitor disease compliance through Poindexter
• Assist with medical questions
• Promote preventive exams and the next onsite visit
• Accept incoming calls on company provided cell
