Health Coach – Wellness Program Coordinator

We are seeking a health coach and wellness program coordinator to provide one-on-one health coaching to employees with their health challenges and assist with coordination with other health providers.

The successful candidate will be a liaison with the CareHere Employee Health Clinic to coach employees on the implementation of their health/wellness prescriptions from the clinic providers. Candidate plans, organizes and directs health coaching, health education and wellness programs to meet county goals and employee needs; assists with collection and analysis of detailed data regarding clinic and program performance to determine return on investment (ROI); identifies health coaching education and wellness goals; plans, implements and evaluates programming to meet these needs; specifies objectives for risk assessment, fitness testing, personalized instruction, ongoing counseling, education, program participation, retention and program evaluation; coordinates with Workers’ Compensation Clinics on return to work restrictions; develops, facilitates and evaluates educational programs and materials pertaining to wellness including Request for Proposal-RFP development and management of outside vendor contracts; develops and implements the Wellness Program marketing plan; provides coaching on lifestyle behavior change education, counseling and support; organizes special events to promote wellness awareness; and manages the employee fitness center including proper use of equipment.

Examples of Important and Essential Duties:

  • Provides one on one onsite health coaching to employees with their health challenges and assists with coordination with other health providers
  • Acts as a liaison with the CareHere Employee Health Clinic to coach employees on the implementation of their health/wellness prescriptions from the clinic providers
  • Coordinates with Workers’ Compensation Clinics on return to work restrictions; including providing support and information about employee wellness to those employees with a work related injury
  • Provides assistance with collection and analysis of detailed data regarding clinic and program performance to determine return on investment (ROI)
  • Manages the employee fitness center including education on proper use of equipment
  • Educates employee groups on healthy lifestyle behaviors
  • Develops new programs
  • Assists with analysis to strategically tie in the Wellness Program to maintain sustainable health insurance plans
  • Keeps employees up-to-date on Wellness Program activities and current health/wellness topics, and educates employees on important health-related issues; assisting employees in becoming more accountable and responsible health care consumers
  • Timely coordinates and schedules wellness programs, classes and events
  • Develops RFP ‘s as needed and manages outside wellness vendor contracts
  • Designs, develops and schedules wellness programs and classes based on employee needs assessment
  • Coordinates all wellness activities including annual Health Fairs
  • Provides support to Wellness Committee
  • Creates and implements Wellness Program Marketing

Knowledge Skills and Abilities:

  • Knowledge of health education, preventative medicine, and wellness concepts
  • Proficient in MS Word, Excel, PowerPoint
  • Ability to engage and gain trust of employees on one on one and group basis
  • Ability to prioritize and manage multiple projects
