Job Description:

  • Target moderate-high risk participants for telephonic and onsite health coaching:
    • Adhere to internal and client deadlines.
    • Limit each consultation to 15 minutes when possible.
    • Review abnormal lab results, inquire about taking medications, discuss health risk factors and encourage healthy lifestyle changes charting all details of conversation with each individual and prognosis throughout the year.
    • Assist all participants with goal setting to encourage health behaviors.
    • Facilitate group review of lab and biometric results.  Collaborate with VP Client Services to create and enhance power point presentations.
  • Field in-bound calls regarding telephonic health coaching program.
  • Facilitate onsite coaching sessions when needed; group and individual.
  • Work in collaboration with other Health Coaches to facilitate and complete physician referral calls.
    • Review all labs immediately following a screening and determine who should receive a phone call with the option to receive their labs early. PR calls should be made within 5 business days of the onsite or off-site screening.
    • Record physician referral activities in Health Park
    • Work in collaboration with other Health Coaches to ensure that physician referral calls are being done in a timely manner.
  • Record all correspondence and continue follow-up with these individuals using Health Park platform.
  • Communicate with Member Services, Technology and Client Services for current information with terminated employees, quarterly Health Challenge, or client event schedule changes.
  • Assist the Director of Screening Services with client events including onsite screenings, group presentations, and individual consultations.
  • Maintain activity report on all telephone consultations and submit quarterly to client and Vice President of Client Services. These reports will indicate all health coaching activity within the client group without identifying names or violating confidentiality.
  • Maintain privacy and confidentiality of health information.

Skills and requirements:

  • Attend training at The Cooper Institute as provided by Sterling Wellness and seek certification as a Healthy Behaviors Coach.
  • Must attend quarterly in-services. Required to complete one Continuing Education session monthly. Continuing education be in the form of webinar, seminar, article, or conference, and does not have to be health/wellness related. Other topics include but are not limited to; computer literacy, leadership, self -improvement or time management. Submit written report to Sterling team.
  • Schedule and maintain evening call hours at least one day per week or as needed in accordance with the call schedule.
  • Ability to provide individual consultations to wellness participants.
  • 40% Travel required.
  • Public speaking required.
  • Flexible schedule to accommodate client events including onsite coaching/education and screenings.
